Our physicians’ schedules fill quickly, so please call as early as possible for the best selection of appointment times. The scheduling staff will work with you to find an appointment time and location that will meet your needs. If you feel your medical condition requires immediate attention, please ask to speak to a nurse. In addition, please ask the staff member about medical information, test results, and imaging studies that you should bring for our physician to review. Bringing all of your pertinent medical information and imaging studies to the first appointment will greatly improve the quality and timeliness of your first consultation. Make sure you identify any specific concerns you would like addressed during your appointment. Referrals are not necessary. We do our utmost to schedule your appointment according to your personal needs as well as your doctor’s.
Phones will only be answered during normal office hours. After hour calls will go to voice mail and returned the next business day. For emergencies, please go to the nearest emergency room or dial 911.

Your First Visit - Patient Registration
Please plan to arrive about 15 minutes early for your first visit so that we can complete your records. Bring your medical insurance card, driver’s license or identification card, and any important medical records, including any recent test results and CT scan films that should be given to the doctors. Please inform us about any health problems, allergies, and medications that you are taking. Please bring all insurance information including insurance cards and, if necessary, your HMO referral. We also re-register returning patients every 12 months, or more frequently if you have new problems or if any information should change. Your co-pay and any outstanding balance will be collected prior to seeing the physician.
To save time during your visit, we encourage new patients to download and print these forms to fill out prior to coming in for your appointment. If you have any questions regarding these forms, our staff will be happy to assist you on the day of your appointment.

We make a sincere attempt to adhere to the appointment schedule as closely as possible. We appreciate the value of your time and dislike long waiting periods. However, patients do not become ill at pre-determined times, and surgery may take longer than anticipated in some cases. Because of this, we may sometimes run behind schedule. If this happens when you are scheduled to be seen, we shall offer you the option to wait, return for a later appointment, or reschedule for another day. Your patience and understanding in these situations is greatly appreciated. We are very grateful for the sympathetic responses so many of our patients have shown under these extreme circumstances.
After Your Visit With The Doctor
After your examination, your doctor may give you prescriptions, instructions, or other recommendations. We encourage you to ask any questions at that time. A follow up visit may be scheduled at the end of your appointment, or you may call at your convenience. Recall reminders will be sent to patients who need follow up care beyond 30 days.
Cancelling or Rescheduling Appointments
Please provide at least 24 hours notice if you must cancel or reschedule your appointment.
Emergency Appointments and Walk-ins
All patients are seen by scheduled appointment. As a courtesy to our scheduled patients, we do not see “walk-in” patients.
If you have questions regarding surgery or need to set a date for surgery, your call will be directed to our nursing staff for scheduling. If our nursing staff is busy assisting other patients, we encourage you to leave a voice mail, and your call will be returned as quickly as possible.

This site features information on tinnitus, or ringing/hissing/buzzing in the ears. It includes current research on tinnitus as well as additional resources and frequently asked questions.

Provides information and support to families of deaf and hard of hearing children.

This organization provides advocacy, education and support for hard of hearing individuals. Their goal is to enhance quality of life for people with hearing loss through resources, technology, annual conventions and local membership chapters.
